As part of the Support at Home program, BallyCara participants may be eligible to access the Assistive Technology and Home Modifications (AT-HM) Scheme - a dedicated support designed to help you live safely, independently, and comfortably at home and in your community. If you're eligible, this scheme provides separate funding from your regular Support at Home budget. Funding is typically approved for 12 months and outlined in your aged care assessment, Notice of Decision, and support plan. You may receive approval for assistive technology, home modifications, or both, with funding allocated under specific tiers depending on your needs.

What AT-HM can help with:

• Equipment and products to support your independence and safety
• Modifications to your home for better accessibility
• Professional prescription or assessment where needed
• Wrap-around services like equipment set-up and training
• Coordination of these supports

Some equipment or modifications - especially more complex items - may require assessment or prescription from a qualified health professional. In some cases, you may be asked to contribute towards the cost.

Once your funding is approved, BallyCara will help to arrange and coordinate everything - from sourcing the right products or modifications to making sure you feel confident using them. We'll work with you and any required health professionals to ensure your equipment or changes to your home are safe, suitable, and tailored to your lifestyle.

The AT-HM Scheme includes a clearly defined list of approved products, equipment, and home modifications designed to support older people to live independently and safely at home. This list - called the AT-HM List - helps ensure that all supports provided are purposeful, safe, and meet nationally recognised standards.
